I worked in mental health and I was so good at documenting, my boss hated me. Because they could never pin anything on me. Always CYA.

The amount of people that don't write everything down is absolutely baffling to me

ESPECIALLY in such complicated, heavily regulated positions, with adjacent positions relevant to quality of care where something going wrong COULD be theoretically pinned on you for failing to handle/treat it as urgently as the other person documents it being.
