It's an entirely different scenario when someone uses a scene from S2 when she has literally no idea about Rem and was just told about her with one in S4 where she's more curious and wants to know about the person she forgot.

If Otto wasn't injured and came with them so much of what Amnesiabaru suffered never would have happened (literal canonical fact stated by Tappei himself lol). Which is why Otto is goated.

It’s mentioned in the novels at least that Ram is still able to feel a basic soul connection to Rem as her twin. So, when she stands next to her, she is able to feel that she is her twin, she just can’t remember anything about it.
I suppose having to take care of your comatose sibling that you don't remember for an entire year would make you attached enough to get that mad about the one guy that remembers her suddenly start saying that he forgot everything. That + the witch's miasma isn't helping.

I know it's been a year, but the last time she was asked to care for comatose Rem, over the awake members, she was ready to let her go and had to be convinced by Subaru that they were sisters. Plus, this is the only major change that happened during that time skip and the series didn't properly address it.
Her last tie to her sister, the one person who actually remembers her claims he does not remember her, when she believes he is an imposter trying to kill them (as shown seconds before he was caged). So she believes that something happened to Subaru and that the imposter
Is trying to erase Rem from the world. All of her reactions up to this point have been in fear and denial that Rem was fully erased from the world.
When Ram has to decide whether they should leave Rem to die in the mansion in arc 4 she doesn't hesitate to run away, but suddenly she pretends to care in arc 6?
“Why does she suddenly pretends she cares?”. You mean after She has spent a year with her by now taking care of her comatose body… Not to mention the connection they have magically… Why would a ram with that experience now care more than a ram with literally no connection?
While Subaru and Ram were traveling to the mansion in arc 4 Subaru spent the entire time (many hours) talking about Rem, which is already more than enough to conclude that this isn't just some random person. Then there's also Rem's appearance that makes it obvious that they're blood related, which Ram sees before Elsa arrives.
In the beginning of arc 6 while Subaru and Ram talk about Rem he starts to talk about the twins relationship once again, but Ram quickly shuts him down (LN Vol. 21 Chapter 1 Section 8):
“Talking about lost memories is all too retrospective. I’m not much of a fan of it.”
She then explains that she doesn't see a reason to talk about Rem before she's awake:
“…If she wakes up and I can remember, then we can talk about the past all we want. And even if I don’t remember, we can still just talk as long as she wakes up.”
This makes her tantrum when Subaru loses his memories extremely confusing, since she obviously doesn't want to hear anything about Rem from Subaru anyway.
There are also multiple similar moments in arc 6 where Ram pretends to suddenly care a lot for Rem when it allows her to criticize Subaru, even tho she obviously doesn't give a flying fuck about Rem.
I'd love for you to explain how any of this could've happened if Ram was interested in the sister she forgot even in the slightest.

It's like the situacion regarding Buzz Lightyear in Toy Story 1. Many laugh at him because he doesn't know he is a toy and operates most of the time under the assumption he is on a secret mission. He did smart choices considering the context he was giving.
