Well, who knows when the new character shows up after the human puts on the headset? Maybe Caine queues them up or makes sure their avatar is perfect for the mind file before spawning them in the circus?
I mean Caine SAYS he makes avatars "perfectly encapsulating your brain files",so you'd think he needs the scan, but maybe he has like, 100 avatars and he gets the closest ones
Why would he make those doors early, but have Pomni's door made after she arrives? Caine even acts surprised when humans arrive and has to make "preparations". For Pomni it wouldn't even be a case of characterse missing her door as hers was right across from Jax and near Ragatha's.

The thing about this series is because it is in a digital world, a lot of production mistakes could have some sort of canon explanation.
