So this is something that can easily be lost in translation, but in the latest chapter in Japanese, Kawaki has started speaking using the kanji for Otsutsuki but with two different furigana pronunciations.
Sometimes he uses 大筒木 (おおつつき, Otsutsuki) and sometimes he uses 大筒木 (ヤツら, yatsura). Yatsura means, roughly, "those lowlifes," though it's often translated as "bastards" just like every other vulgar version of "those guys" in Japanese, and he's now grouping in not just the Otsutsuki, but every threat associated with them.
In other words, "Ohtsutsuki" is becoming the word he thinks of in his mind when referring to any of those whose existence threaten the peace Naruto built, include Code, the Divine Tree Beings, and both Boruto and himself.
My translation for his monologue, trying to keep to official Viz style, would be something like:
Kawaki: "There are still so many jerks I need to kill."
Kawaki: "The Ohtsutsuki, and..."
Kawaki: "...that whole pack of animals tied to 'em..."
Kawaki: "The peace you tried to build from the ground up..."
Kawaki: "...and the ruin that could tear it down..."
Kawaki: "Momoshiki, Code."
Kawaki: "The Divine Tree Beings..."
Kawaki: "Those [Ohtsutsuki] lowlifes keep bringing ruin to our doorstep."
Kawaki: "That's just what the Ohtsutsuki are."
Kawaki: "Until we wipe out every last one of those [Ohtsutsuki] lowlifes..."
Kawaki: "...this world will never know true peace."
Kawaki: "That's why I'll kill 'em."
Kawaki: "They all need to be wiped out."
Kawaki: "Anyone who's already turned into an Ohtsutsuki..."
Kawaki: "...including Boruto and me."
