Obviously this would be very stupid of this person, but legitimate question (not trying to be inflammatory, honestly curious).

I sort of assumed a medical chart would explicitly include if someone was transgender/had transitioned, is that not the case? I guess the hysterectomy does that itself, but it feels like it would be something explicit to ensure the doctor doesn't overlook it when glancing at a chart? Because wouldn't that be potentially relevant to diagnosis/treatment? Obviously if you're like...there for a broken bone or at the dentist it's whatever, but if you were actually sick?

Doctors can't read charts on a normal day. I've gotten prescribed medication that I'm explicitly allergic to, it was only the pharmacist who caught it.

That's  why a very good doctor once told us that every single time someone (including herself) was about to prescribe some medication for our kid we have to mention the specific drug intolerance the kid has. Because parents/patients usually know their issues quite well and tend to not forget then. Because they only have a single patient they care for and not thousands.

Hell it's why whenever I get a new prescription I always make sure to remind them that I have had a stroke since there are a lot of medications that you should not take if you've had one

Oh seriously? I've got to remember to mention this whenever my daughter gets a prescription. Thanks for this!
