I think they are actually a 4chan trans girl. I remember going through that account, and they know too much about trans culture not to be trans themselves. In fact, they are part of a circle that is likely a bunch of 4chan Trans people, as there are a few recurring people they interact with who share the same sense of humor.

Misty Orthodox, otherwise known as Michael Orthodox, is a twitter user who was originally famous for obsessively retweeting transgender folks' selfies and being an average right-wing snark about it. The *OTHER* thing that Orthodox is famous for is the infamous tweet in which Orthodox says something to the effect of "I'm Trooning Out!", to which a twitter user named IsraelFan or something similar with a profile picture of Adolph Hitler replied "Michael Orthodox please lock in."
Ever since, Orthodox has been progressively getting more transgender(?), from changing their Profile Picture to that of Jax from the Amazing Digital Circus (a canonically transgender fictional character) to changing their twitter display to "Misty Orthodox" to even posting a picture of an estrogen injection/pill they've gotten (Which one exactly I cannot remember). Ever since, pleading for Misty Orthodox to "lock in" has become sort of a joke.
It's still debatable whether Misty Orthodox is just a troll, and if so who exactly the joke is on. Is the whole account a parody of Trans-obsessed right-wingers who are transphobic in an attempt to block out their true selves? Or is it a joke on left-wingers who are so empathetic as to fall for one of the oldest tricks in the book as a sort of psy-op?
I, in all my hope and good will for humanity would genuinely like to believe that Orthodox is transgender and just needs to sort some things out, but otherwise I shall leave the judgement up to you.

I think he’s just really good at baiting people. If you look at any of the more recent tweets (he’s gone back to Michael) he seems to regularly flip between being ostensibly supportive of trans people (sometimes correctly gendering them and everything) which ticks off his orthodox bro followers and then sometimes going back to right wing or general Christian posts
Edit: he recently screenshotted this exact post and commented the following on his account:

I was born with the body-shape of 'rectangle'; my hips and waist have about an inch between them. From the front, I'm about as curvy as a brick. I've been mistaken for a chubby teen boy before.

Also estrogen does redistribute fat to the hips so at a similar body fat percentage trans women on HRT for long enough will not look different from cis women in this regard. But also individual variances exist among all women and also not every woman has body fat levels meeting incel beauty standards...

Yeah, it definitely shifts around fat and muscle plenty, my arms are thinner and my fat is more around my hips now. There's totally plenty of variance and parts of the bell curve for different features overlap between the sexes, I've seen cis women with very rectangular builds and hips that were not very defined and I've seen cis men with wider hips and narrower waists. Kind of weird that some people seem to deny that especially considering how one trait is height and everyone has seen a woman that was taller than a man

The hip line and hip to waist ratio is not solely determined by fat distrulibution. While those on hrt for long enough can develop a silhouette that is more traditionally feminine, the barrier a lot of them may see is in the width of the iliac crests, and their shoulders.

Most women do not look like Jessica Rabbit. Most women don’t have 20in waists and 45in hips. This person looks like plenty of cis women. She is extremely passing.
It’s a great example of how transphobia is also misogyny, because the standard guys like this apply to trans women is, like, a “1% of the population” ideal. It’s 10/10 celebrities and models. They act like “passing” and being S-tier hot are the same thing; “she doesn’t look like Sydney Sweeney, thus she’s clearly a man.”
But, like, the world is filled with women who don’t look like Sydney Sweeney. The cashier at the Dollar Tree doesn’t look like her, and she’s passing. The woman in line at Panera Bread doesn’t look like Kendall Jenner, and a normal person would still see her as a woman. The very premise that “valid femininity” is something random men get to decide, based on a standard of “super hot from a straight male POV,” is a huge fucking problem for all women.
