Confusion Over DoDMERB Form

was just reading a post here about their DoDMERB, and it got me thinking about my own one.

I just printed out my 16 page medical history from DODMETS, and now i’m confused. The whole form looks like it’s going for the medical exam, so what forms do i bring for the optometrist exam?

Is there a part of the 16 page thing that doesnt go to the doctor, or am i just missing something?

That just for the medical. You don’t bring anything form for the eye exams.

Nothing. No forms needed for eye exam.
